Before
you come
To help us with the diagnosis and treatment of your pet, we will
ask you to fill out a registration form and questionnaire before
your appointment. This form is available for you to download as
a PDF or
RTF file.
You should then print the file and complete the form prior to your
arrival. We will also need certain medical records from your veterinarian.
Your veterinarian may send them to us, or you may be asked to bring
them with you on your first visit.
Fees
You will receive an estimate of anticipated costs before major diagnostic
and therapeutic procedures are performed. Payment is due on discharge. We only accept Visa and MasterCard,
Cash and Checks. No American Express, Discover or Care Credit.
Special instructions
We will be happy to see your pet when his/her skin is in any condition
or when he/she is on any medications for any skin problem. However,
certain factors may affect our ability to fully assess your pet.
Thus we request the following:
| |
Please do not feed your pet after midnight the night before
your appointment. If your pet has diabetes please call for instructions. |
| |
Please do not bathe your pet for at least 4 days prior to your
appointment. |
| |
Please do not apply any topical medications to your pet's skin
or ears for at least 2-3 days prior to your appointment. |
| |
If you are interested in intradermal skin testing for your allergic
pet, corticosteroids (topical or systemic) should not be given
or applied for at least 4-6 weeks prior to skin testing. Be
aware that many ear and eye medications contain steriods and
should not be used. Anti-histamines should be withdrawn for
at least 2 weeks prior to skin testing. Blood allergy testing
is available if your pet cannot be without corticosteroids or
anti-histamines. Antibiotics, antifungals, antivirals, heartworm
preventative, oatmeal containing products, flea products, cyclosporine
for KCS (dry eye), thyroid medications, antiseizure medication,
and heart medications should be continued as normal. Any medications
that are necessary to maintain life should be given to your
pet. |
| |
If possible, bring a list of medications your pet has received
in the past. The list should include when the medication was
given, for how long and at what dose. If it is easier, bring
medication, shampoo, ear cleaners, etc. with you. |
To
provide the best care and in order to obtain a complete dermatologic
history and examination, we will require your presence for 1-2 hours.
In addition, you should plan on having your pet stay with us for
several more hours, at least for the initial evaluation. We will
work with you and your schedule as best we can.
